Requests for information are best made using our online secure service called OneTrust. This service can be used to manage your request from start to finish and can speed up the request process.
Paper copies of these forms are available on request however, wherever possible, we would encourage you to use the online OneTrust forms.
OneTrust provides a secure portal from which you can download copies of the requested information once the system informs you the records are available.
In some circumstances we may use the Scottish Wide Area Network (SWAN) Secure File Transfer (SFT) service for sending your records instead of, or in addition to, using the OneTrust portal. The SWAN SFT relies on the MOVEit platform which uses data centres in the EU. As such, when SWAN SFT is used, your personal data will be transferred outside of the UK.

Please note that you do not need to use any of these forms in order to make a Subject Access Request; for instance, you may also make a request verbally, by email or in a letter. However, the forms have been designed to make it easier for you to provide us with all the information that we need to verify your identity and comply with your request. For this reason we would recommend using, or referring to, the relevant form.
